Navigate Here: Utilities > Templates > Dynamic Document Templates > click the Add icon 
You can add a dynamic document template on this screen.
To add a dynamic document template, fill in the following fields:
The required fields are noted with an asterisk (*).
Entity Name*: The entity in which the dynamic document template will be associated with. Clicking 
will show the entities which are available for selection.
Template Name*: Enter a name for the template. It must be unique.
Description: The description of the template.
Is Default: Select this checkbox if you want to set this template as the default template for the selected entity. Only one template within the same entity can be set as the default template.
Click the Save button to save the new dynamic document template. The system will go to the Browse Dynamic Document Templates screen.
Click the Cancel button to go to the Browse Dynamic Document Templates screen without saving.
